A shared approach to implementation

The Murrumbidgee Living Well, Your Way implementation team is a cohesive team, underpinned by collective leadership. MLHD and MPHN team members have a strong shared identity, and work together across both organisations to deliver key initiatives.

Living Well, Your Way have adopted a unique, cross-organisational approach, bringing together team members from both MLHD and MPHN. This team have a sense of collective ownership and a shared vision that has fostered a strong partnership between the organisations. This collaboration has been instrumental to the effective implementation of the initiative. Having the opportunity to cohabitate space at both MLHD and MPHN sites has enabled more effective and efficient realisation of implementation activities.

Caroline Holtby

Director, Collaborative Commissioning (MLHD)

I lead the Living Well, Your Way initiative in the Murrumbidgee. My role is to work with stakeholders across our region in acute and primary care to deliver improved outcomes for people with chronic disease. This work is important because it is about access to quality care for our patients. By working together, we can make a difference.

Kelly Dal Broi

LWYW Senior Project Officer (MPHN)

My role is focused on engaging with primary care and the local community to support the implementation and outcomes of Living Well, Your Way across the Murrumbidgee.

I'm proud to be part of the program focused on making improvements in our health systems to assist patients with chronic health conditions with achieving better health outcomes and quality of life. 

David Hardiman

Senior Lead (MLHD)

As Senior Lead, I support the implementation and evaluation of the Murrumbidgee Collaborative Commissioning initiative ‘Living Well, Your Way’. This initiative aims to provide early intervention strategies that result in an improved experience for both chronic patients and clinicians, leading to better outcomes at lower cost.

 I am passionate about breaking down barriers within the health system to ensure our community have greater access to timely care, closer to home.

Daniell Taylor

Change Manager (MLHD)

My role is focused on implementing and managing health-related initiatives, programs, and projects to improve outcomes for patients with Chronic Respiratory and Heart Failure conditions in the community. As Change Manager I work collaboratively with clinicians and managers throughout the region, developing relationships with the Murrumbidgee Local Health District and Murrumbidgee Primary Health Network to drive successful adoption and sustained utilisation of health-related changes.

Donna Stevenson

Change Manager (MLHD)

As Change Manager for the Transition to Home initiative my role is to support our frontline staff at Griffith Base Hospital implement and sustain the changes needed to optimise access to care for acute and non-admitted patients. The focus includes providing patients access to the resources they need, when they need them, and in the way they choose to use them. The initial priority will be delivery of integrated health care to patients with Chronic Obstructive Airway Disease and Chronic Heart Failure.

Elise Penton

Practice Nurse Project Officer (MPHN)

My role as Practice Nurse Project Officer to collaborate with practice nurses across the Murrumbidgee to help provide the best care and education for our community members living with chronic disease. 

I am passionate about supporting our nurses to achieve their goals and work at the top of their scope to support our patients to live well within our communities.

Qudratullah Azimy

Monitoring and Evaluation Manager (MPHN)

My role is to coordinate with teams across MPHN and MLHD to ensure data collection tools and reporting methods are established and the key performance indicators and outcome measures for Living Well, Your Way are met and demonstrated.

I am very excited to be part of the team that works to support people in our community to live a better life.

Kym Ramsey

Community Pharmacist (MPHN)

Kym Ramsey has taken up the role in Collaborative Commissioning team as the Community Pharmacist working on the Pharmacy Screening Pilot. Kym has worked as a pharmacist for over 20 years and is passionate about primary and preventative healthcare. He loves working with people to improve their health outcomes and has a particular interest in home medication reviews. Kym is located at MPHN’s Griffith office at St Vincents’ Private Community Hospital working Wednesday to Friday.
