The Outreach Respiratory Clinics aim to provide timely and accessible care for early detection, assessment, and management of chronic respiratory conditions—improving long-term outcomes and quality of life for patients.
Comprehensive respiratory care close to home
These clinics deliver community-based respiratory screening, assessment, and diagnostic testing for at-risk patients referred by their GP due to persistent complex respiratory symptoms, comorbidities and/or those patients who experience frequent hospitalisations.
Referred patients receive:
A thorough respiratory health assessment
Spirometry and other lung function testing
Self-management education and smoking cessation support
Specialist review and ongoing care planning
Care is coordinated with the patient’s local GP, providing guidance and education on evidence-based management of chronic respiratory conditions such as Chronic Obstructive Pulmonary Disease (COPD).
Clinic Locations & Specialist Support
Clinics are held once per month at targeted locations across the region (see below for a list of scheduled clinics). These clinics are supported by:
MLHD Chronic Respiratory & Heart Failure Team – both clinician and administrative support
Respiratory Physicians
Funded under COAG 19(2), this service is facilitated through the Living Well, Your Way initiative.
By bringing respiratory expertise closer to home, the clinics ensure earlier diagnosis, more effective intervention, and better long-term support for individuals living with chronic respiratory conditions.
