Building a better way of delivering healthcare for people living with chronic conditions such as Chronic Obstructive Pulmonary Disease (COPD) and Congestive Heart Failure (CHF) in the Murrumbidgee.
Living Well, Your Way
Living Well, Your Way (LWYW) was a Collaborative Commissioning initiative between the Murrumbidgee Local Health District (MLHD) and Murrumbidgee Primary Health Network (MPHN), proudly funded by the NSW Ministry of Health.
It aimed to build a better way of accessing and delivering healthcare for people with chronic conditions including Chronic Obstructive Pulmonary Disease (COPD) and Congestive Heart Failure (CHF) and keep people healthier at home for longer.
The LWYW pathway was a model of care with an integrated approach inclusive of acute, community and primary care, along with public and private healthcare providers across the Murrumbidgee. The Pathway supported existing providers to enhance their services, and formed partnerships to fill identified gaps.
