Our Activities

  • Winter Strategy

    GPs supporting proactive care for patients with chronic disease at risk of deterioration over the winter period.

  • Pharmacy Screening Program

    Aims to proactively identify patients with potential heart and lung conditions, then refer them to their general practice for further assessment, testing and potential diagnosis.

  • Care Monitor

    Digital healthcare to manage chronic disease and empowering patients to be proactive in maintaining their health.

  • Outreach Heart Failure Diagnostic Clinic

    Visiting specialists and clinicians visit rural communities to provide testing and diagnostics for patients suffering from symptoms of heart failure.

  • Nurse Led Clinics

    General practice nurses run clinics focused on key areas of need for their community to provide targeted care for vulnerable patients.

  • Caridac and Pulmonary Rehabilitation

    Exercise and education programs available for people diagnosed with chronic heart and respiratory conditions.

Rapid Access Clinic 

Seed funding from Living Well, Your Way has been used to expand the WWBH Rapid Access Clinic (RAC) to offer out of hours services on evenings and weekends.

The RAC provides an alternative place of assessment, treatment, and care coordination for triage category 4 and 5 patients with urgent and non-life threatening conditions. 

The RAC is a referral only service. This includes transfer of care from the emergency department, as well as accepting referrals from general practice, NSW Ambulance, and residential aged care facilities. In 2025, referrals will be expanded to include community pharmacists.

Conditions appropriate for RAC referral can include:

  • Lower urinary tract infections and uncomplicated pyelonephritis

  • Cellulitis

  • Pneumonia

  • Exacerbation of COPD/CHF

  • Symptomatic drainage of ascites

  • Catheter/peg care

  • Wound care

  • Urgent and non urgent blood transfusions

  • Iron infusion

  • Anti-D injection

  • Replacement of gastrostom/jejunostomy

Living Well, Your Way

Building a better way of delivering healthcare for people living with chronic conditions such as Chronic Obstructive Pulmonary Disease (COPD) and Congestive Heart Failure (CHF).

Living Well, Your Way (LWYW) is a Collaborative Commissioning initiative between the Murrumbidgee Local Health District (MLHD) and Murrumbidgee Primary Health Network (MPHN), proudly funded by the NSW Ministry of Health.  

It aims to build a better way of accessing and delivering healthcare for people with chronic conditions including Chronic Obstructive Pulmonary Disease (COPD) and Congestive Heart Failure (CHF) and keep people healthier at home for longer.

The LWYW pathway is a model of care with an integrated approach inclusive of acute, community and primary care, along with public and private healthcare providers across the Murrumbidgee. The Pathway supports existing providers to enhance their services, and forms partnerships to fill identified gaps.
