July project update
Our team has been particularly busy this past month as we transition from the Joint Development Phase (JDP) to the Implementation Phase and roll out the initiative in the Aboriginal Health organisations and communities across the district.
Mawang Gaway meeting
Mawang Gaway is a Wagga Wagga based Aboriginal consultative group run by local Elders and community members. We recently had the opportunity to present our care pathway to the group and have meaningful discussions around issues facing local Indigenous people and how we seek to address these issues and improve health outcomes through the Living Well initiative.
We are pleased to have been invited to attend Mawang Gaway monthly meetings where we will continue discussions and work together to ensure the Living Well pathway meets the needs of Aboriginal people.
Tumut Rehab (Restoring Function)
While COVID-19 and long-standing staff vacancies in the area put an end to face-to-face rehabilitation sessions in the Tumut region, Living Well has recommenced the Pulmonary and Heart Failure Rehabilitation Service (Restoring Function) at Tumut Health Service. Kicking off on 28 June, this new-look service will see Tumut trialling a new Physiotherapy-led model expected to be rolled out across the Murrumbidgee region.
The program aims to improve the individual’s health and equip them with the knowledge and support they need to confidently manage their condition and feel better in their daily lives.
It includes an individually tailored plan to link in with community services and provide strategies for managing acute symptoms, multidisciplinary education and information sessions about self-managing health at home and in the community, and an 8-week supervised group physical activity program.
Currently five patients from Tumut and Batlow are working with the service, attending two sessions each week where they are supported by a qualified Physiotherapist and an Allied Health Assistant.
