It's our birthday!

“Remember to celebrate milestones as you prepare for the road ahead.” ~ Nelson Mandela

This month marks one year since first launching Living Well. It also marks a significant step forward in our journey as we progress to the Implementation Phase of this initiative. If running to schedule is any indicator of success, then we are taking the win!

We’re pleased to report things are progressing well with the Living Well initiative and with each small victory we grow confident of the important changes to healthcare we are making here in the Murrumbidgee in caring for people living with chronic conditions.

We look forward to continuing this journey with you!

As we celebrate Living Well’s first birthday, our team shares their thoughts on the initiative, all that has been achieved so far and hopes for the future.

Caroline Holtby our Program Director:

Living Well, Your Way is a large-scale reform that focuses on people and the impact chronic disease has on their whole life, rather than just managing their condition. With Living Well, we’re looking to improve the way we deliver healthcare in our region in ways that matter to the people that live here. So far, I have really enjoyed working with people from across the region and being able to share the innovative work Murrumbidgee is doing at a state and national level. I can’t wait to see and share more stories of impact.”

Sue Massey, Senior Lead Collaborative Commissioning:

“We are committed to listening to our communities, clinicians and patients to ensure we plan for the best ways we can deliver care to people equitably and holistically. Living Well, Your Way is our chance to make a meaningful difference to the way we look at chronic care in regional areas. I have loved engaging with communities to understand what is important to them. I am committed to working very hard toward improving outcomes for people living with chronic disease in our region and ensuring our clinicians are supported and engaged in doing so.”

Ryan Williamson our Change Manager:

Living Well is important to ensuring people in rural areas have the same access to high level care as people living in big cities. The most rewarding part for me is getting out and meeting people living with chronic conditions and community groups across the region, learning about their experiences and providing them with opportunities to have a say in creating a service that will really make a difference here.”

Melanie Reeves our Senior Project Officer:

“It’s vital that we work to improve the patient's journey and not allow our rural location to affect the standard of care we provide. The most rewarding part for me so far is the development of the care pathway and working through how we can collaborate with various stakeholders to improve or support systems, and to achieve better outcomes for people living with chronic conditions.”

It’s not always easy to understand all the work we’re doing. This diagram paints a thousand words about the Living Way pathway and all its moving parts.
