Project focus: Implementation Phase begins
Living Well has reached an exciting and critical point in its journey, with the Joint Development Phase (JDP), or planning part of the initiative, rapidly drawing to a close and implementation beginning.
As the Living Well team wraps up the final steps of the JDP, MLHD Chief Executive Jill Ludford and MPHN CEO Melissa Neal are preparing to present the pathway to the Secretary and Deputy Secretary of Health NSW. Slated for the end of July, this pivotal meeting will include discussions about the sustainability of the model and securing the necessary seed funding to support the program over the next three years.
The first part of the Implementation Phase is already underway with the Respiratory and Heart Failure Model of Care being rolled out by the newly appointed Clinical Respiratory and Heart Failure Nurse Clinicians and an updated model of care being used in the recently refreshed Restoring Function (Pulmonary and Heart Failure Rehabilitation) service.
As we move further into the Implementation phase, more models are set to be introduced or expanded across the region.
Living Well Program Director, Murrumbidgee Collaborative Commissioning with MLHD and MPHN, Caroline Holtby said that reaching this milestone is a big achievement for the entire Living Well community and she is excited for the next phase.

People living with chronic conditions in this region are in very good hands and I truly believe the Living Well initiative can have a profound impact here. We look forward to working with all of our Living Well partners over the coming months to introduce further changes and monitor and evaluate outcomes.”
