April project update
Our team has also been busy working toward a number of important steps, including:
Living Well Clinical Advisory Group – This group is now established and meeting monthly. At this months’ meeting, the group will review outcome measures for Living Well initiatives and discussed the Chronic Heart Failure Pathway in more detail.
Costings – As part of the Collaborative Commissioning Joint Development Process, a sustainability analysis was undertaken to determine when the model would become “self-sustaining” from a financial point of view. According to the analysis, self-sustainability will be achieved when the benefits of the model (in terms of avoiding hospital admissions/ED presentations) outweigh the costs of implementation.
Respiratory and Heart Failure submissions – Two submissions are currently with the Patient Centred Co-Commissioning Group (PCCG) awaiting endorsement:
Griffith Respiratory and Heart Failure Support Service which includes: a nurse specialist; public multidisciplinary outpatient clinic; and pulmonary and heart failure rehabilitation programs
MLHD Respiratory and Heart Failure Clinical Service Model of Care, including a Murrumbidgee-wide Pulmonary and Heart Failure Rehabilitation Model.
