May project update

Our team has also been busy working toward a number of important steps, including:

  • Lumos rollout
    The Lumos program is part of the Ministry of Health’s move to support value-based healthcare and is funded by the Commonwealth Government under the Health Innovation Fund.  It is a new and ethically approved program that sheds light on the patient journey through New South Wales by securely linking encoded and de-identified data from general practices to local health district data across the state. So far, 25 practices in the Murrumbidgee district have agreed to take part in the program, which will provide the Living Well initiative with relevant, localised data to map and improve the journey for people living with COPD and CHF.

  • Care navigation app
    The Living Well team has commenced work with a technology provider to develop a patient-centred application (app) that will help patients on the Living Well Pathway to better manage their care and navigate the health system.
    The app is designed to streamline communications between patients and care providers, simplify monitoring of conditions and provide education to patients, which will facilitate their continued care in the community and empower them to self-manage their health.

Cristy's unique career has taken her from country NSW to the city lights of Clarendon Street South Melbourne and back again. With an early career in radio as a copywriter and creative strategist, she is now a Jill of all trades as a graphic designer, website builder, blog writer, video editor, social media manager, marketing strategist and more. 

In fact, give her any task and this chick will figure out how to do it! Go on, we dare you!

No, really, we DARE you!!

Cristy has won two Australian Commercial Radio Awards (ACRAs) for Best Ad and Best Sales Promotion, and even has an 'Employee of the Year' certificate with her name on it.

Cristy and her husband James have traveled extensively through Russia, China and South East Asia, and have two fur-babies, Sooty (cat) and Panda (puppy). Cristy loves drinking coffee, meeting people to drink coffee, coffee tasting and coffee flavoured cocktails. She also enjoys road trips, TED Talks and watching cat videos on youtube.
